Ontario's decision to integrate private clinics into its healthcare system could be a game-changer. With plans to add 100,000 MRI and CT scans annually, this initiative aims to significantly reduce wait times.

However, the expansion of private clinics has unsurprisingly, become quite partisan as policymakers debate a potential slide towards a two-tiered healthcare system.

My worldview has always been to trust but verify. Given the dire situation we find ourselves in, all well intentioned, bold initiatives should be tested.

There also may some positive knock on impacts for private companies who have historically been blocked from entering the Ontario market because of the inability to access MRI and CT scan licenses.

Warren Buffett once famously remarked, "Only when the tide goes out do you discover who's been swimming naked." I don't think there could be a more appropriate quote for the current state of healthcare services rollups; specifically within physician practice management groups (PPMs).

For the past decade+, PPM rollups have been a hot topic in the broader investment landscape, riding the waves of economic conditions that have fluctuated from favorable (ZIRP, cov-lite leverage etc.) to challenging (interest rates > organic growth, return of covenants etc.).

As the tide recedes, we're starting to get some visibility into which strategies are truly sustainable and which were, metaphorically speaking, "swimming naked".

This article is my attempt at:

  1. Explaining how the PPM rollup craze came to be;
  2. Assessing the pitfalls that many investors fell into; and
  3. Offering a framework for how to evaluate rollup opportunities going forward
