I'm #hiring for a key role on the Clover Assistant product team - a Lead Clinical Data Product Manager!
This role sits in a critical place on the team, working to bring administrative, clinical, and social determinants of health data together - not for the purpose of a massive CDR, but rather for use by PCPs. We pull data together to generate a clinically intuitive way for PCPs to engage with the data they need for early identification and longitudinal care management of chronic conditions in the medicare population.
Even better, it works! We've already shown the impact in our patient populations at scale from this combination of data. Our CKD white paper (https://lnkd.in/evUiGyAT) highlights how that data has helped lead to the early identification of CKD in our population, resulting in earlier treatment and the attenuation of the decline in kidney function. This role sits at the intersection, bringing medication claims & orders, medical claims & charts, lab claims & results, specialist documents and more into a single clinically coherent framework to help PCPs.
