It's well-known that inadequate resorption into the veins can lead to #hydrocephalus. But only recently shown that pathological excessive resorption can occur at #csfvenousfistula, typically located at a thoracic nerve root. This recent #digitalspinalmyelogram image we captured, of contrast injected through a lumbar puncture needle escaping into a spinal segmental vein, is one of the best I've seen. You might call it an "empty cyst sign" because the leak is so high flow that contrast never pools in a #tarlovcyst that certainly exists. The patient went on to have transvenous embolization of the segmental vein with consequent resolution of her disabling positional headaches.
