The very act of preventing a failure in a prototype can massively increase the cost and timeline of bringing it to market. It can be far cheaper and faster to lose the first few prototypes, iterate rapidly, and field a far superior product - all the whole exercising your production system.
Risk is prevalent in all aspects of life. A financial portfolio that is never allowed to lose money will massively underperform a balanced risk portfolio in the long term.
NASA and traditional players should take note of this, and remember the 1950s-70s when they themselves embraced this ethos before risk-aversion set in.
