Why do so many docs feel stuck in the rat race of hitting RVU targets?

Dr. Singh recently published an editorial in Ophthalmology Blue Journal entitled, Transactional Care and the Looming Glaucoma Public Health Crisis

Despite high per capita spending, the US lags in health metrics due to systemic issues including financial pressures on physicians. For some, these pressures have shifted focus from long-term patient outcomes to more immediate financial gain per unit time, thus indirectly affecting patient treatment options. This is evident in the glaucoma field, where innovations favor mild over severe disease, driven by market forces.

"Another possible, but not well-studied, cause of declining health of the United States population is the increasingly transactional nature of medicine. Although the growth of social media has led to a logarithmic increase in the number of interactions between individuals, the tradeoff has been a decline in the number of relationships defined by a meaningful shared future. Transactional relationships such as those between customers and mechanics or accountants, where one party pays the other a fee for a service based on a contractual agreement, remain highly prevalent in our society. In contrast, nontransactional relationships, where individuals help others without any guarantee that the favor will be repaid, make up a progressively smaller proportion of all relationships. It has been pointed out that such nontransactional relationships, exemplified by a college student helping an ill roommate or a neighbor coming to the aid of someone in distress, account for fewer of our regular interations now than in past years.

Thus, it is not surprising that some in society may not understand what a nontransactional relationship looks like. Although the practice of medicine in the United States has long been transactional, the financial pressures on physicians, amplified (if not created) by insurance companies and private equity groups, has pushed the physician–patient relationship to look more like the one between a mechanic and a car owner, rather than one between two neighbors. This cannot be good for our health! Ideally, the physician–patient relationship should be transformational, rather than transactional, based on mutual respect, emotional investment, trust, shared values, and the common goal of improved health." -excerpt from Kuldev Singh editorial https://lnkd.in/gvZjcegZ #transactionalmedicine
