Another opportunity to join us!
Vidya Ashley and I are looking for an experienced RN Manager to join our delegated care management leadership team!
As a full-risk primary care group responsible for our patients’ clinical and financial outcomes, Oak Street Health works with managed care payers to assume “delegated” care management responsibilities. This allows us to develop high-quality care plans in collaboration with the patient’s PCP, improve patient experience, and reduce unnecessary costs - both for patients and for the healthcare system.
Now that we’re part of CVS Health, we’re exploring opportunities to improve care plans and health outcomes for even more patients than before. DM us for more details or feel free to share directly with your networks!
