Cigna just announced two expanded partnerships with value-based kidney care providers.

Cigna patients in at least 27 new markets will now have access to pre-dialysis kidney disease management through an expanded partnership with Healthmap Solutions. These programs are currently available in Florida, Georgia, North Carolina, and South Carolina. They aim to improve clinical outcomes, reduce ER visits and hospital admissions, identify care gaps, and provide monthly patient-specific recommendations. [1]

For patients on dialysis, Interwell Health and Cigna just expanded their existing programs (since 2017) for patients being treated at Fresenius clinics. This new expanded program will now include patients not previously covered under value-based care contracts. According to the release, Interwell’s program will coordinate appointments and transportation, provide coaching, and implement a risk-stratification process to reduce hospitalizations and improve patient outcomes. Back in early 2017, Cigna launched its first value-based arrangement with kidney disease specialists as part of Cigna Collaborative Care. Under the initiative, Fresenius was paid for the kidney dialysis services it provided to its Cigna patients, while its affiliate, insurer Fresenius Health Partners, assumed separate responsibility to manage medical costs and improve patient outcomes.

A few years later, a partnership with then-Cricket Health expanded on Cigna's CKD case management and pre-dialysis offerings. They focused on providing education, support and personalized guidance to Cigna customers with kidney disease between doctor’s appointments, including PCPs and kidney doctors. [3]

According to an internal Cigna study of Cricket Health customers from May 2020-June 2021, among enrolled Cigna customers, the first year of the collaboration included more than 7,000 clinical interactions, 1,000 peer mentor interactions, and 2,250 views of educational content and videos.

Enrolled customers saw a more than 45% reduction in unnecessary hospitalization.

Additionally, 100% of those with early stage kidney disease enrolled in the program made a choice other than in-center dialysis, and 63% began treatment in an out-patient setting.

By early 2022, the industry's largest VB kidney care entity came together through a 3-way, $2.4B merger to form Interwell. Cigna Ventures was among the leading investor groups in the round, alongside familiar kidney care & VBC backers like Oak HC/FT and Blue Shield of California. [4]

Last summer, Healthmap raised $100M from its lead investor, WindRose Health Investors, another familiar VBC name with its investments in CardioOne and Curation Health. [5]
